A plan for getting back to your day.
Use follow-up visits to turn a treatment plan into clear, practical steps for home, work, and everyday life.
Request an appointment

Recovery can feel abstract when the only question is whether you are better. Bring a real-life goal to the appointment: sitting through a meeting, managing stairs, or returning to a usual household task. A specific goal gives you and your clinician something useful to discuss at the next visit.
Make the next steps easy to understand
Before leaving, ask what you should do, what you should avoid, and how long to follow the instructions. Confirm whether another appointment, test, or referral is needed. If you are unsure how to arrange it, ask the office who handles that step.
If more than one clinician is involved, keep their contact information in one place and share relevant reports through the clinic’s approved process. Avoid sending sensitive medical records in an ordinary email.
Record patterns, not just difficult moments
A short note about what you did, how symptoms responded, and any questions can make a follow-up more useful. This is a conversation aid, not a diagnostic test. Report new or worsening symptoms promptly using the instructions from your care team.
Talk about real-world constraints
Work schedules, childcare, travel, and transportation may affect how you manage appointments. Let the clinic know what is difficult. Ask about appointment timing and whether an alternative arrangement is clinically appropriate; do not assume remote care is suitable for every injury.
Keep clinical decisions with the treating team
